If I have done ANY procedure on you in the past, in order to be able to take your consent forms/procedure history with me to my new location, you HAVE TO sign this paperwork transfer approval form. I need for everyone to do this because without your procedure history I will not know what pigment mixtures I used on you, important details about your healing process and all the notes I took about you and your skin in the past. Download the form below and all you have to do is print it out, sign/date it, either scan it into your computer or take a picture of it and then email ( [email protected] ) or text it back to me to my new phone number ( 812.760.1035 ). For those that may have problems downloading the file I have also placed an image of the form that you can screen shot and print out if you'd rather do it that way.☺️ If any of those are an issue for you, please get ahold of me (email/text/call), and I will be happy to supply a hard copy that you can sign in person.☺️ YOU ABSOLUTELY HAVE TO SIGN THIS FORM FOR ME TO BE ABLE TO TAKE YOUR PROCEDURE HISTORY/CONSENT FORM WITH ME TO MY NEW LOCATION.
